Terms and Conditions

Residents are accepted on the following terms and conditions and on admission an agreement is required to be signed by the service user or their legal representative or such persons being empowered to sign on behalf of the service user

1. THIS AGREEMENT is between Elmfield House and THE SERVICE USER.

2. Occupancy

  • Long term
  • Respite
  • Intermediate / Rehabilitation

The first four weeks of admission shall be regarded as a trial period for the benefit of the Service User and the Home.

3. Fees include:

  • Full board and lodging
  • 24 hour care
  • All meals, cooked to special dietary requirements if needed
  • Laundry (excluding dry cleaning)

Fees do not include: Hairdressing, chiropody, newspapers, dentistry, physiotherapy, opticians, toiletries, outings, escort duties and transport.

The fee is to be paid one calendar month in advance. The management will give one months notice in advance of any proposed fee increase.

The fees are payable one month in advance and will be payable on the same day each month by standing order or direct debit.

4. The Home is registered as a care home by the C.Q.C. the Care Quality Commission , which is responsible for ensuring that standards are maintained. If an occasion should occur where a complaint or query arises the Service User is referred to Elmfield House which has a written procedure for dealing with complaints.

If the complaint is not resolved then the Service User may wish to refer to the inspection team of the Care Quality Commission (CQC) whose address is:

5. Termination of agreement:

A Service User leaving and terminating this agreement must give one calendar months notice in writing or one calendar months fees in lieu of notice. A similar notice period will be given by the management. In the event of the death or an immediate change in needs to nursing care of the Service User, fees will be due for the 2 weeks following the removal of personal effects from the room allocated and any monies overpaid from that due date will be refunded by the company.

The management may give notice to the Service User to leave the Home under the following circumstances:

  • Non payment of fees
  • If having consulted the Service User and taken advice from appropriate members of the primary healthcare team e.g. G.P, district nurse or social worker, concerning the present and future needs of the Service User, the management are no longer able to meet the Service Users needs
  • Any circumstances in behavior which the management feel may be seriously detrimental to the Home or the welfare of other Service Users

6. Temporary absence:

A Service User who is temporarily absent for any reason will be charged normal fees due to the fact that the room and their property retained inside their room remains exclusive to the service user.

7. Personal effects and mobility:

  • Service Users are free to journey out alone. However, the Home cannot accept responsibility for the Service users safety while away from the Home under any circumstances
  • All electrical items brought in by Service Users on admission or during occupation of the Home shall be inspected to ensure their safety prior to use
  • At the discretion of the Home items of furniture may be brought in by the Service User subject to inspection by the Home as to condition and defects liable to render the article unsafe or unfit. Transportation insurance and eventual removal of such items shall be the Service Users responsibility or that of the Executors
  • The Home will not be held responsible for the loss of any laundry that is not clearly named. No responsibility will be taken for lost or broken spectacles, dentures or hearing aids

8. Insurance:

Elmfield House has a Care Home Insurance policy which covers the Service Users personal effects up to the value of £500. In the event of a claim there is a fee of £50 excess which will need to be recovered from the Service User. Any items which are valued over of the sum of £500 Service Users are requested to insure them on a separate policy from the Care Home policy.

9. Smoking is not permitted in the building. Service Users, staff and visitors are requested to smoke outside in the back garden or at the front of the house.

10. The management reserves the right to transfer the Service User to any other room after consultation with them and their relatives.

11. The procedure on the death of the Service User will take in to full account their known wishes or those of their family as notified at the time of admission or at any later date.

12. Management does not normally take responsibility of any personal finance for the Service Users monies unless designated as Appointee. It is therefore requested that relatives take full responsibility of monies regarding the Service User.
